Job Description:

The Clinical Services Director is responsible for the management of the WelbeHealth clinic, social work, behavioral health services, and rehabilitation services and ongoing clinical guidance and support to the Interdisciplinary Team (IDT). The Clinical Services Director is a member of the WelbeHealth leadership team and facilitates collaborative relationships across WelbeHealth teams and stakeholders to promote the WelbeHealth values, team culture, and mission.
Duties and Responsibilities:


  • Responsible for the delivery of high-quality WelbeHealth clinic, social work, behavioral health services, and rehabilitation service.
  • Provide ongoing clinical guidance and support to the IDT as well as provide back-up planning and facilitation of the IDT meetings as needed.
  • Manage clinic, social work, and rehabilitation operations including hiring, training, supervising and mentoring staff and ensuring clear and effective workflows.
  • Responsible for ensuring WelbeHealth participants experience smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.) deploying staff as appropriate and collaborating closely with the Complex Care Team, Center Director and Medical Director.
  • Responsible for achieving key performance indicators as outlined in monthly operating review and weekly tactical meetings.
  • Responsible for compliance with PACE, ADHC and DHCS regulations inclusive of audit prep, monitoring non-compliance and implementation of corrective action plans.
  • In collaboration with the Medical Director and central support team, ensure PACE program is achieving clinical excellence objectives and key results.
  • In collaboration with the PACE PCP, ensure all orders are carried out as directed.
  • Collaborates with the HR Operations Team when a workplace injury or illness takes place.
  • Support mission expansion and participate in the intake and enrollment process to ensure prospective participants move through the enrollment process in a timely and effective manner.
  • In partnership with other leaders, facilitate open communication and effective problem solving that supports high staff morale and excellent participant experience.
  • Maintain appropriate level and scheduling of staff to ensure safe and effective care.
  • Manage department budget.
  • Responsible for all activities relevant to services furnished, including the development of personnel qualifications and the assignment of personnel.
  • Manage direct reports including hiring, training, supervising and mentoring.
  • Be legally authorized to practice in position location, required.
  • Only act within the scope of his or her authority to practice.
  • Other duties as assigned. -

    Qualifications and Requirements:

    • -Bachelor's Degree in relevant field; or an additional three years' of relevant Management experience may be substituted.
    • Master's Degree in relevant field preferred.
    • Graduate of an Accredited School of Nursing: Bachelor of Science (BSN) preferred.
    • Unencumbered California Registered Nurse (RN) License.
    • Valid State Driver's License with a clean DMV record.
    • Copy of recent Vehicle Insurance required.
    • Minimum of four years' experience within the last five years in a, outpatient care clinic or health facility.
    • Knowledge and skills to treat participants and manage complex clinical needs.
    • Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
    • Three years' supervisory experience with demonstrated ability to mentor and develop team members.
    • Multi-disciplinary management experience (i.e., nursing, social work, rehab.)
    • Value Based Care experience preferred.
    • Ability to process accurate information in a timely manner.
    • Excellent leadership, organizational, customer service and communication skills with seniors, their families and interdisciplinary team members.
    • Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects.
    • Highly motivated, inspirational leader who is able to innovatively engage, support and remove barriers for the team to ensure the team consistently meet established goals.
    • Ability to work independently with minimal supervision.
    • Demonstrated ability to prioritize in a fast-paced environment.
    • Experience and competency working with people from diverse backgrounds and cultures.
