Key Responsibilities
Manage community and resource coordination activities, ensuring
members are connected to local resources and care management
Oversee and approve tasks for the community and resource
coordination team based on requirements and member needs
Partner with team leaders to ensure effective care and compliance
with state and regulatory guidelines
Required Qualifications
Bachelor's degree required
5+ years of related experience required
Equivalent experience may be considered
Experience in managing community resource coordination
preferred
Knowledge of state and regulatory guidelines related to care
management
