Job Description San Gabriel Convalescent Center is currently looking for a Registered Nurse (RN) Supervisor to care for our patients and facilitate their speedy recovery. You will be responsible for educating them and their families on prevention and healthy habits. $5,000 Sign On Bonus New Grads Welcome Shifts Available: (SNF) Looking to Fill Immediately FULL-TIME: (NOC) 11:00PM-7:00AM / SNF San Gabriel Convalescent Center is looking for a qualified nursing supervisor to oversee patients and manage the nursing team of our facility. Our ideal candidate has solid experience as a registered nurse and is able to handle stressful situations. This position requires team management skills and a problem-solving attitude. Further training in management or administration would be a bonus. Ultimately, you should be able to ensure smooth nursing operation at our facility through day-to-day management and long term planning. This position involves 90% direct patient care and 10% administrative non-direct patient care. The Registered Nurse assists in providing a clean, safe, dignified, happy and healthy environment for residents by performing the duties as described Responsibilities Include but are not limited to: Schedule nurses--- shifts Assign nurses to patients Ensure nursing operational standards are met Provide reports on productivity, quality and customer service metrics Ensure all nurses follow policies and procedures Train new team members Evaluate nurses--- performance Inform family members about medical procedures and doctors--- instructions Handle complaints or other issues Establish a compassionate environment by providing psychological support to patients, family and friends Keep records of nurses--- files, patients--- cases and procedures Complies with the facility---s attendance policy requiring regular, reliable, and punctual attendance Makes actual patient rounds, assessing and observing the following at least three times per day: Monitoring vital signs as necessary with appropriate interventions as indicated Positioning Restraint application Hydration/nutritional status/feeding program o Actual patient interviews o Assessment of new admissions Pre/post-dialysis assessment Monitoring lab and x-ray values related to patient---s condition o Incontinence and related interventions Skin care and/or changes of skin condition Use of proper assistive devices Ensures timely response to call lights Pacemaker check Abnormal laboratory values o Identifies/assesses emergency medical situations and provides appropriate interventions as indicated Acute changes in condition: nausea, diarrhea, infections, LOC changes, confusion, elevated temperature Pain management/intervention Assess behavior and provide interventions as indicate Wound treatments Therapy treatments Incidents; falls, etc. Documentation and follow-up with timely notification of physician and family Care planning as needed Evaluates patients for bowel and bladder program Intravenous Insertion, site changes, dressing changes, intravenous medication administration, assessing intravenous insertion site for s/s of infection and/or need for changes RN may be assigned to administer medications and/or treatments if necessary Establish and implement patient plans of care and document care provided appropriately Insures that Medicare guidelines as well as HMO/PPO guidelines are within the facility Medicare change of condition documentation. Performs administrative non-direct non-patient care as assigned by the DON and/or Administrator Monitors condition changes and properly documents and follows-up as necessary. Implements measures to prevent and reduce incontinence for residents, including: Initiates within two weeks of admission of an incontinent resident, a written assessment by a licensed nurse to determine the patient---s ability to participate in a bowel and/or bladder management program. Creates an individualized plan, in addition to the patient care plan, for each resident in a bowel and/or bladder management program. Provides a weekly written evaluation in the progress notes by a licensed nurse of the resident---s performance in the bowel and/or bladder management program. Skills Proven work experience as a nursing supervisor In-depth knowledge of hospital procedures Thorough knowledge of legal regulations and best practices in healthcare Team management skills Great communication and people skills Excellent organizational and problem-solving skills BSc or diploma in nursing; training in healthcare administration is a plus Valid nursing license Must be Fully Vaccinated If you would like to schedule an Interview Call Miriam 626-399- 4552 San Gabriel Convalescent Center 8035 E.
