youth, families, adults, and older adults. We offer a full range of

services, housing, continuing adult education and early childhood
education programs to Medi-Cal eligible individuals and families.

Stanislaus, Stockton, Tulare and Ventura.POSITION SUMMARY:Under the
general supervision of the Program Director of the Older Adult
Field Capable Clinical Services Program provides non-billable
peer-to-peer support to Program clients as requested by Program
Director or designee. Such activities may include friendly visitor,
telephone reassurance, accompanying consumer to appointments or
activities in the community, interpreter services and similar
activities when other resources are not available.ESSENTIAL DUTIES
AND RESPONSIBILITIES:Works actively within a multi-disciplinary
program environment to develop and deliver peer-to-peer support for
Program clients.Under direct supervision, works actively with the
Program team to develop strategies for outreach to clients who tend
to isolate themselves.Under direct supervision, provides supportive
assistance to, and modeling of self-efficacy with the client to
help him/her obtain and keep housing, health care, SSI, Social
Security and other benefits, as appropriate.Under direct
supervision, provides supportive assistance to, and modeling of
self-advocacy with the client to help him/her research and access
needed and desired community resources.Encourages and supports
client's skills and talents.Works cooperatively with others to
assure the smooth and seamless delivery of comprehensive services
to members.Under direct supervision, completes documentation and
other paperwork in a timely and accurate manner and in accordance
with MHA standards and funding source requirements.Maintains
compliance standards in accordance with HIPAA, State, County, and
Agency guidelines to ensure client confidentiality, protected
health information (PHI) and quality of care.Attends and
participates in staff meetings to provide input towards program
development and staff training, and greater understanding of
consumers' issues and needs.Reports to work on time and maintains
reliable and regular attendance.Models Pacific Clinics' approach,
mission and core values in all communication and
correspondence.Communicates effectively in a culturally competent
and diverse consumer population and promotes favorable interaction
with managers, co-workers and others.Performs other duties as
requested.EDUCATION and/or EXPERIENCE/POSITION REQUIREMENTS:High
school diploma or GED preferred.Position is reserved for an
individual who has been functionally disabled by a psychiatric
illness for at least six (6) months during his/her adult life. If a
dually diagnosed mental health consumer, must be more than one year
in recovery.Basic typing and computer/word processing
skills.Flexible work schedule to include occasional evenings,
weekends and holidays.Knowledge of local community
resources.Ability to communicate effectivelyMust possess a valid
California driver's license and maintain an insurable driving
record under the Clinics' liability policy (if driving two more
times per week on company business) OR if driving is not required,
demonstrated ability to use public transportation, if
applicable.PHYSICAL DEMANDS:While performing the duties of this job
the employee is frequently required to stand or sit. The employee
is required to use hands to produce records and/or documentation in
manual or electronic format. The employee must regularly lift
and/or move up to 5 pounds and occasionally move or lift up to 10
pounds.The physical demands described here are representative of
those that must be met by an employee to successfully perform the
essential functions of this job.
